The community and outreach service shall aim to reach out to individuals who may face barriers to accessing traditional healthcare settings and offer support, treatment, and resources in community-based settings. The division shall strive to strengthen/establish the following community and outreach services
Outreach and Engagement Service
Outreach and Engagement service to reach out to individuals who may not actively seek help for their mental health or substance use concerns
Assessment and Screening Programme
Assessment and Screening programme to identify mental health and substance use concerns
Case Management and Care Coordination Service
Case Management and Care Coordination services to ensure individuals receive comprehensive and integrated support
Peer Support Service
Peer Support Services to provide understanding, empathy, and practical guidance to individuals seeking support
Community and Outreach Services

Medication Management
Medication Management in collaboration with healthcare professionals, for individuals requiring pharmacological interventions
Education and Prevention Program
Education and Prevention Programs with a focus on promoting mental health and substance use awareness through educational initiatives
Referrals
Referrals and linkages to help individuals access additional support beyond the scope of community-based services
